Ambulatory by design

Surgical intervention delivered at the outpatient center — no hospital admission required.

Our care model
01 / Context

Why outpatient surgical access matters for complex wound patients.

Patients with chronic wounds face fragmented care, long referral waits, and avoidable hospitalizations. Delayed surgical intervention is a leading driver of infection progression, emergency admission, and lower-extremity amputation — particularly in underserved communities.

85%

Amputations preceded by a foot ulcer

Most preventable with timely wound and surgical care.

5year

Mortality after major amputation

Exceeds that of most solid-organ cancers.

Disparity in amputation rates

Medicaid-insured and Black patients versus commercially insured cohorts.

$17B

Annual U.S. spend

Diabetic-foot complications — largely preventable.

Our position
Our surgical center provides timely, coordinated access to interventions that support healing and keep patients in the community — closing the gap between bedside wound care and the operating room. Jonathan Johnson, MD · Surgical Director
02 / Care Model

One practice. One team. One continuum of wound care.

When advanced intervention is needed, patients transition seamlessly from bedside or in-office wound care to our co-located outpatient surgical center — ensuring continuity, eliminating referral delays, and avoiding unnecessary hospitalization.
